Detecting $500M in Medicare Fraud with Public Data
A single Go binary ingests 22 CMS datasets (548 million rows), scores 1.47 million Medicare providers across 10 years of history, and serves an interactive investigation dashboard. 42% Precision@100 — an 890x improvement over random selection. Average detection lead time: 6.9 years before OIG exclusion. Built in 13 days by 4 people. Infrastructure cost: under $800/month.
What We Built
- 27 fraud detection features across billing volume, procedure mix, drug prescribing, payment networks, sanctions, geographic, graph analysis, and enrichment
- Three-way ensemble scoring — weighted composite, ML corroboration (Isolation Forest, K-Means, LOF), and gradient boosted trees with focal loss (AUC: 0.965)
- 82.6M-edge provider graph — six custom network algorithms detecting prescribing isolation, opioid contagion, and exclusion proximity
- Live investigation dashboard — heatmaps, provider profiles, radar charts, real-time weight adjustment for investigators
Out-of-Sample Results
- 37 confirmed fraud cases found in top 250 — providers never in training data, flagged purely from billing patterns
- $500M+ total fraud value — including a $250M opioid conspiracy, $14.3M wound care fraud, and 55,000 false claims on deceased patients
- Single binary, zero dependencies — DuckDB embedded, GraphWizard in-process, all assets compiled in. No Kubernetes. No data lake.
- GraphWizard open-sourced — 40+ graph algorithms for Go, built to replace a 100GB Memgraph dependency
